How to create HIPAA-Compliant Contact and Intake Forms Using Hushmail

This guide walks you through how to add HIPAA-compliant contact forms and intake forms to Jane Websites, using Hushmail. Adding secure forms are the best way to protect patient information while making it simple for patients to get in touch or submit intake paperwork online. 

 

How to add HIPAA-compliant contact form

You can add a Hushmail contact form to any existing page on your website, such as your Contact page. These steps walk you through embedding the form using a Custom Code block.
 

1. Navigate to the page where you want to add your contact form, such as your Contact page.
2. Click the Add Block button on the left hand side of your website, and select the Custom Code Block option. 
3. Once the Custom Code Block is created, click the pencil icon to edit the block.
4. Copy the embed code from your Hushmail account and paste it into the Custom Code block.
5. Click the Update Block button to apply the changes and display the form on the page. 

How to add HIPAA-compliant intake form

Most clinics want their Intake Forms on their own page. That page can also be password-protected if you want to limit who can access your intake forms. The steps below walk you through creating a new page and embedding your Hushmail intake form.

1. Click the “+” Quick Add button in the top-right corner and click Page. Alternatively, go to Admin > Content, then click the Add Page button.
2. Add a Title for your page (e.g., “Intake Forms”)
3. Scroll down and add a Subtitle if needed. You can also choose whether to password-protect the page as well.
4. Click the Save Page button once you’re ready. 
5. Now you'll be on your new Intake Forms page. From here, you'll click the Add Block button on the left hand side of your page, and select Custom Code. 
6. Click the Update Block button to apply the changes and display the form on the page.
